The Belgium Death Certificate is called a Extrait d’Acte de Décès/Uittreksel van de Overlijdensakte which is issued by Officer of Vital Statistics (Officier de l'Etat Civil/Burgerlijke Stand). The doctor who confirms the death will issue a death certificate. The death will then be reported to an officer of the state (Officier de l’Etat Civil/Ambtenaar van de Burgerlijke) at the municipal offices of the commune where the death occurred. 
 

Belgium Marriage Certificate 

 
The Belgium Marriage Certificate is called a Extrait d’Acte de Mariage/Uittreksel uit de Huwelijksakte which is issued by Officer of Vital Statistics (Officier de l'Etat Civil/Dienst Bevolking). Only a civil marriage ceremony is legal in Belgium with a marriage certificate being issued. Only full marriage certificates are acceptable for passport purposes. All marriages are registered and recognised by individual communes, there is no central registry.

The Belgium Divorce Certificate is called Extrait d’Acte de Divorce/Uittreksel van de Scheidingsakte where the issuing authority is Clerk of the Lower Court (Greffe du Tribunal de lere Instance/Griffie van de Rechtbank van Eerste Aanleg).
