The Ukraine Birth Certificate is called Svidotstvo pro narodzhennya (Свідоцтво про народження) which is issued by District or Regional State Registration of Civil Status Offices. Births should be registered in Ukraine within 1 month and birth certificates are issued following the registration, on application. These birth certificates are issued by the local registration offices. Children born though a surrogacy arrangement will be issued with a birth certificate containing the names of the commissioning parents.
Ukraine Death Certificate
The Ukraine Death Certificate is called Svidotstvo pro smert (Свідоцтво про смерть) which is issued by District or Regional State Registration of Civil Status Offices. All deaths must be registered within 3 to 5 days. Death certificates are issued from the local or district registries in Ukraine, following the notification of a death.
Ukraine Marriage Certificate
The Ukraine Marriage Certificate is called Svidotstvo pro shlyub (Свідоцтво про шлюб) which is issued by District or Regional State Registration of Civil Status Offices. Marriage certificates are issued by local marriage offices in Ukraine. The records of marriage are held in a civil registrations book. There are two types of marriage certificate in circulation:
a former Soviet Union marriage certificate (issued until approximately 1992)
a Ukrainian marriage certificate (issued from 1992 until present)
Ukraine Divorce Certificate
The Ukraine Divorce Certificate is called Svidotstvo pro rozirvannya shlyubu (Свідоцтво про розірвання шлюбу) where the issuing authority is District or regional State registration of civil status offices.
Ukraine Identity Cards
The Ukraine Identity Card is called Паспорт громадянина України (Passport of Ukrainian citizen). The LПаспорт громадянина України (Passport of Ukrainian citizen) is Compulsory for citizens 14 and older. Before 2016, the national identity card was a blue soft paper booklet like the typical international passport. In 2016, the government began issuing credit-card-sized biometric identity cards containing an RFID chip. The bearer's address is not printed on the card but is instead coded on the chip and can be accessed by any NFC compatible device. The card is printed in Ukrainian and English except for patronymic information that is only printed in Ukrainian. Cards last ten years before expiring, except that minors' identity cards must be renewed upon turning 18.
Is Ukraine a member of the Hague Service Convention?
Ukraine is a signatory to the Hague Service Convention (Convention of 15 November 1965). Ukraine objects to service under Articles 10(a), 10(b), and 10( c ) within its territory.
